Hello All,
I need some help in answering some questions on the DS-156. I am getting ready to apply for my and my wife's revalidation and was wondering how to respond to these questions on the form. Please help and share your thoughts

Here are the question numbers I need help with:

22- When you intend to arrive in the US?

25- Name and Tel number of the person you will be staying with?

26- How long do you intend to stay in the US?

27- Whats the purpose of your trip?

28- Who will pay for trip?

29- Have you ever been in the US?

30- Have you ever issued a US visa?

35- Names and relationships of persons travelling with you

Please share info on how you answered these questions when applying for revalidation.

22- Already in US
25 - Own Address
26 - Until the visa exipres
27 - Work for *** in compliance with the approved H1B
28 - Through co. salary and personal assets
29 - Yes (Would be advisable to attach a sheet with your stays in US)
30 - Yes. I'm assuming you got a visa somehow to enter the US.
35 - Wife.

------ I am in US ( your wife will also write the same)
25- Name and Tel number of the person you will be staying with?
---- I am in US, write your wife name your home address and telephone # ( your wife will write your name and your home address and home ph#
26- How long do you intend to stay in the US?
-----------check your H1 approval notice and write the H1 expiry date ( your wife will write the validity of her H4 extension or her latest I-94)
27- Whats the purpose of your trip?
working in US on H1 status ( your wife will write to be with my spouse)
28- Who will pay for trip?
--- I am in US( your wife will write the same)
29- Have you ever been in the US?
---YES( your wife will write the same)

30- Have you ever issued a US visa?
YES( your wife will write the same)

35- Names and relationships of persons travelling with you
- spouse in US with me( your wife will write the same)

** you have also to fill up form DSI57 and in item 3 write your name
with black pen in your native laguage like Hindi, Tamil, Gujarati, Punjabi, Telgu ( if you are from India) your wife need NOT to fill up form DS I57

in the Fedex envelope send prepaid Fedex enelope and that Fedex prepaid envelope for return of your passports have ADDRES

From:
your name your address
TO,
your name your address

** for the fedex payment use the credit card that has validity of 5-6 months
 
Q's # 22 in DS156 form

Wht shld I write for when do you intend to arrive in US? If you are already in US.
Do I need to put my last US entry date ?

Please advice.

Thanks for the resp Ginnu, but as per 156 form it can only take a Date format. I'm filling 156 thru Online.

I don't have a option of writing "I am in US".

So my q's was can I put my last entry date?.

Please advice.

----- one can take the print out of form and can write with black pen

------ I will leave this blank and will attach sheet A and will explain on it and will also write on the cover letter please see attached SHEET A and my answer to Item XYZ ( one must sign the attached sheets with name and date on it
So my q's was can I put my last entry date?.
--- I dont think it is correct
