DS-156 Form only one day left ..Please help

Hi,
Can anybody please let me know if we just leave the fields blank or
write none or N/A(when trying to use N/A the National Identification number field does not take "/")on the DS-156 form when one is going to mexico/canada
for all the fields which are irrelevant to the person filling the form.

Please advice whether to leave the fields blank or write none or N/A.

Thanks a lot for your response ginnu; I just had this question

b'coz on the mutyhy.com site http://www.murthy.com/news/n_deptst.html

it says that in all the DOS applications forms for names none or N/A shud not

be used and the ques 8 & 9 which are other surnames used ,Other first and

middle names used should be left blank or instead can I use none since I

have only one first and last name and no other surnames.
