Document needed to apply for AP

Can any one tell me the documents needed to file for AP at local INS office. Can I mail the documents?.
I am trying to apply at the Los Angeles district office.

The Form is I131, need 2 photos, check and a letter explaining why you need AP.
Also see the link on field offices to check whether you can apply with the ASC, instead of Service center
