you shouldn't need to, since the validity of the medicals are extended while your petition is pending.

I thought medical is only good for a year...
yes, I am sure. REad up on extending the validity of I-693. There have been numerous USCIS memos about it.

yes it is good for one year so, you will have do it again but instead of mailing it to USCIS, keep the sealed envelope and take it with you for your interview , i'm told 60% of the time they will ask you and tell you to mail it if all goes wel then the'll approve u're case.just wondering if i have to do my medical again for my second interview.since it is one year apart.

The OP already gave the Sealed Envelope to USCIS before his first interview. A nowadays, you MUST mail it along with you I-485 application. You can not bring it with you to the interview, they will RFI you for it.
