5. CIS alleged that on October 16, 1987, I signed Form I-690 attesting that I had knowingly used false material information to obtain entry into the United States.
I-690 is a waiver for inadmissibility. It sounds to me that Catholic Services didn't explain to you the full extend of the form and you signed it without knowledge of what it was for. In the process , according to USCIS you have provided false information. Without knowing what you falsely provided on the form, USCIS denied your application. The onus is on you to prove to USCIS that you didn't know what you were signing for, something very difficult to prove if there was no attached declaration from Christian Services that they helped you fill out the form.
With all the inconsistencies in your story it's no wonder why USCIS denied your case.
