Changing jobs before vs after H1 status ends

Hello,
My 6th and final H1 year will end in April 2008, and I am still waiting for I485 approval for more than a year now (probably stuck in name check, which could take a much longer time).
In general, what are my options for the next step? Do I really need to file H1 extension at this point?
In particular, I am weighing the options for changing jobs before versus after April 2008. Which is recommended and why?
