Birth Certificate question

Does the birth certificate issued by local authorities needs to contain the name of both the parents or just one is sufficient (e.g. father only) ?
For those who claim that both are needed, can you point me to a credible resource ?

U need to support the BC with a affidavit from the parent(s) so that information like parents, date/time of birth, city/town country etc. are given (that is what a birth certificate is!!)
You will be giving the names of parents in 485/G325 Dont you think you need to support all the data presented in those forms with some documentation?
