Probably what happened is that their data entry people didn't link your case number or A number to her application.My wife case is pending still.....
What does this mean? Are they requesting anything from us? Thanks.

Type up a letter explaining the history of your situation, listing all the details with case numbers for your I-140 and both of your I-485's, and your A-numbers. Emphasize that her I-485 was filed more than 5 years ago and yours was approved more than 1 year ago.
Make two copies of the letter and a copy of your I-485 approval and her I-485 receipt and go for an Infopass appointment. Summarize your situation verbally, and show them the letter and ask what is happening to her case. Give them one letter to keep and show the supervisor, and ask them to send the second copy to the Service Center.
Then send the third copy along with another copy of your I-485 approval notice and her I-485 receipt by registered mail to the Service Center. If your Service Center is Texas, send it here:

Then wait a month or two. If hers is not approved yet, write your Congressperson and Senator. And if that doesn't work within a month or two, file WOM.
