Hi y'all, I have a multiple-entry B1/B2 visa with an annotation that reads: "To stay for 4 months for vacation, will not seek employment or adjust status." What if I happen to receive a job offer? Can I still adjust status? Or maybe extend my stay? Pls help, lotsa thx...

That annotation is a clear sign that you are on thin ice to start off with. The consul could have easily denied you a visa. Consider yourself lucky that you earned the consul's trust. So repay the trust and leave after 4 months of vacationing. If you receive a job offer, ask them to sponsor you for H-1B and wait it out in your country of normal residence.

Annotations stand for strict rules and you need to abide by it. No extension will be provided on annotated visa.
