Applying for EAD extension but I have doubt.

I am about to file extension for EAD. I know the list of documents
to be attached.

I have doubt about column 11 in I-765 form.

11. Have you ever before applied for Employment Authorization from INS?

If I say "Yes" , there is column Date(s)
What date should I put there? Should I write Receipt date of EAD?
