Advice on new i-130 & g-325a

Greetings!

I am looking at my I-130 & G-325A forms to have my alien wife join me me in the U.S (I am US citizen). There are two G-325A forms required for this. I have questions about filling in the fields at bottom of the G-325A forms.

1) Should I check "OTHER (SPECIFY):" and enter "I-130" on both G-325A forms?

2) Do I sign both forms or do I send hers for her to sign?

3) Do I place my name on both forms in the "APPLICANT:" box at bottom or do I put her information on her form? On her form do I put "N/A" for "Alien registration number" or do I leave it blank?

P.S. Do I leave "FILE NUMBER" blank onthe forms? Do I send both passport copy and birth certificate copy OR is either one alone OK?

Status as Permanent Resident.

2) Do I sign both forms or do I send hers for her to sign?

She has to sign hers.

On your form - you are the applicant. On her form, she is the applicant.

On her form do I put "N/A" for "Alien registration number" or do I leave it blank?

If she has A#, write it in. If she doesn't, leave it blank.

Do I leave "FILE NUMBER" blank on the forms?
If she has A#, write it in. If she doesn't, leave it blank.

Do I send both passport copy and birth certificate copy OR is either one alone OK?

Either one.
 
A few more questions on g-325a

I realized I have three more questions about my g-325a form.

1) If I need more room in the Last 5-years Resisdence list where do I include the additional addresses?

2) If I lived outside the US for 1.2 years, but lived at 2 different residences, each for about half that time, do I put anything in the "address outside the US for more than 1 year" area? I.e. do I combine them and enter them on that one line?

3) Is there any special way to abbreviate city names that can't fit in the space provided on the form? I.e. Saint Petersburg, Los Angeles..?
