AC21 letter foramt...Ginnu & others.. please help

I want to file AC21 and I need some info:

What is the procedure?

Do I need to write a letter? If so, is there a good format that I can follow?

Also, I'm moving from Georgia to California, where should I send my ac21 application?

Don't we need a copy of labor and I I40 with the AC21 letter ?

Thank god , since I don't have a copy of LC and I140 I was wondering How to file AC21 .
