Sorry,not an answer to yur question. I am not clear what exactly you mean when you say "By the time she is ready to travel, her AP will expire". Had the I-485 been not approved for your wife, how would your wife have come back to the US if her AP expired? I mean on H1 or some other NIV?

It should have been included with the filing fee for I-485 to begin with.

I also noticed that the website was not all that clear when you file EAD & AP applications together (as to which is the principal application). PO Box 87485 - is a NO, since that is for I-485, and you use it when you file 485/EAD/AP applications together I myself never applied for EAD & AP...

I guess so, as long as you have your official names there (rather than baptismal name), the certificate is in church stationery (translate if needed). You never know what exactly CIS wants. so it wouldn't hurt if you send any marraige registration also.
