This is required which shows the proof that u filed tax returns to IRS regularly. only W2s may be enough but it\'s always better send tax returns. So far I learned from the IRS web site, it\'s couples of hrs of work if u go to ur local IRS office and fill the form and get the previous returns...

Previous Tax returns If you yet not file 2001 tax returns and better to file e-file immediately and apply to IRS with form 4506 for the previous 3 years tax returns i.e., 1999,2000 and 2001.

My labor was certified and lawayer was saying that he didn\'t get the 750 form with the certified letter. Is it OK he can apply with the certified letter for 140. Thanks in advance.

Nice to see you too ... In my case it was approved on the next day. I got it in 7 business days.But unfortunately I got only the certified letter with DOT code without the attached 750 form after now my lawyer sending a request to NY dol again things are delaying about 15 days to apply for 140...
