Place where you have to go for stamping was noted on the approval notice. In San Jose, you can join Appointment line [separate line]. Go in the time bracket mentioned in the approval notice. Take 2 ADIT photos without shadows, AP [if any], EAD [if any], passport, and...
Clarification
You should always keep copies of the tax returns you have submitted. So for example, if you are in the US for the last ten years, you must have copies of last 7 years returns. If you are in the US for only two years, obviously you will have only two returns.
If you have filed the returns online or through HR Block you may be able to get copies from them. I believe you are required to preserve copies of your tax returns for the last 7 years.
A small risk
There is a very small risk that if the case is approved, deferred inspection should be granted at port of entry. This is equal to the risk that AP is not accepted because of other reasons.

RFE was for birth certificate, 2001 tax return, W2 and 3 pay slips.
I only avoided checking AVM this week! Prior to that the message was 160 days.
Got approval in the mail for self, wife and kids.
