There is no wait period to apply for GC once the parents are in U.S as they fall under the immediate relative category. I have applied for my parents in less than 3 weeks after they came to the U.S and they have received their GC Approval. See my signature below for more info on the timeline.

I would recommend bringing your mother back through a Non-Busy Port of Entry where officers might be more lenient (For ex: Newark,NJ , philadelphia,pa..) on the GC Flap . You shoud definitely avoid ( JFk, Chicago, Detroit, LA, SFO)

Overstaying is not an issue for Immediate Relatives Category ( Spouse , parents ...). Please use the I-130 Approval to file the I-485 and attached documentation to the USCIS Chicago Lockbox.
Go to a different city/district to get a NABC !! You can show them the Notarized Affidavits and they will issue a NABC . The Affdavits will serve as the official date of birth. What is the native country issuing NABC ?
File for your mom's GC (I-130/I-485 Concurrent) about a couple of weeks after she comes here to the U.S. Once the I-485 is filed she would not need to apply for extension since she is in legal status as long as I-485 is pending or GC is approved.

I would not recommend using any handwritten W-2 forms. Keep her income as "0" if you donot have any proof of income like " Tax filing , official W-2 forms , 1099 ..." . You can also go to the local IRS office to request a Tax Transcript for the last 3 years which can be used in lieu of the W-2...
