The H1-B document you have (valid until 2/2009) authorizes you to work in the US legally and shows your status. The H1-B visa that you recieve in the passport (based on this document) allows you to enter the US when coming from abroad.
But Why go through the hassle of getting a visa stamped...
If I am not wrong, you can re-enter the US using your AP, and still maintain your H1-B status. There's been a few threads discussing this. That way you have no need to goto the US consulate to get a new passport stamped visa.

I would recommend that you do mention your earlier visit, though I am not sure if they would have records going back 20 years. If you do get asked about it, show the visa copy. I doubt they will ask for your earlier passport but incase they do, mention you were unable to find the passport since...
