For ur 1st question, you should state Adjustment of status.
I'm not sure about the 2nd question, since you havent used ur EAD. If EAD expiry date is latter than the H1b expiry date, it would be logical to give that, in my opinion.

You may want to take up the W-2 job as GC is only for future employment. GC sponsoring employer might come into picture only if there's a employment verification.
If it's more than 6 months since you applied for 485, you may want to consider AC-21...
Apply for EAD asap

It's better to apply for EAD atleast 90 days before expiry and you can apply even 6 months before expiry. Since yours is due to expire in May, you should apply immediately.

BCIS goes by notice date
As far as i know, BCIS goes by Notice date. If you read the postings of people who have spoken to IIO, you'll notice them mentioning that BCIS goes by notice date.
You have to be very patient when it comes to GC. Your turn will come soon.

RFEs could be sent for employment verification, medical clearance, address verification, relationship to primary applicant etc
Or something that could be totally new.

Spoke to IIO today, who was very polite. Luckily got to speak to her after 25 mins of wait.
I checked the following and here are the reponses:
Change of Address: Sent about a month ago but it was not updated in the system. Gave her the new address.
EAD renewal: RD 02/11/2003...

INS goes by notice date and October must have been a heavy volume month. Since your notice date is in October, it might be taking long.
My notice date is Feb 02 and the FP took about 4 months.

INS should be notified within 10 days of changing address. You need to fill form AR-11 and submit it.
You should be able to find the AR-11 pdf in INS website

INS says that if EAD is not renewed within 90 days from the date it was applied, an interim EAD can be obtained from the local INS office.
