My wife had this problem 3 years ago and now things might have changed as discussed in this thread. We got our letter from us consulate at abdullah haroon road karachi and she had to go personally for appointments and to pick up the letter, but this was before 9/11 and now things have changed...
I think it should be obtained from Karachi, because they have a cosular section there, I don't know the precise location, because they moved after attack on consulate which used be on Abdullah Haroon road. I don't know if you can obtain letter by mail?. May be somebody knowledgable comment on it.
Yes I think you don't have to, because special registration was meant for people who were present in usa. Upon exit you will surrender your I-94 which was the proof of sp registration. Upon your reentry you will be examined by an immigration officer who will take all your info and thats all.

Travelling on AP and coming from pakistan is a big deal. They will not let you board the plane until you have transport letter from US embassy. This letter takes anywhere from 4 to 6 weeks. So be careful and plan accordingly. This happened to several of my friends. Once you have the letter, its ok.

My case was transfered to Tampa, Florida on 9th July 2002 with ND 4/2001 and I have not heard anything from TSC or local office yet. Generally speaking local transfer further delays the processing by several months and not considered a good sign.

You can apply for 485 regarless what you choose at the time of 140, but it will take a little longer. applying for 485 has no bearing for ead, your question is not very clear about that, you can get ead via 485 only, ead is not given for cp cases.
