Just an FYI Hi guestusa , From what little knowledge I have, I140 is the employer application and if you change the employer, then your process starts again. Only when I140 is approved and I485 has been filed for over 6 months, we can use "Portability" option, to change employer, if hired...

Just an FYI Hi jonboutilier, You can do all your vacciantions and Medical tests (Skin Test / Check X-Ray) from your primary care physican with your Co-Pay (If you have HMP). Then take all these results to the INS doctor. That way you will end up paying less. Thanks.
