Now that that AC 21 guildelines have been published it is normally taking 8-10 weeks for VSC. Earlier all such cases were on hold for about 6-7 months.

I agree with prabha. The extension varies depending on where you are in the GC process.

I think You can still apply for 7th year extension even after I-140 is approved for CP. In my case my I-140 was approved for CP but I changed my mind later to AOS. Applied for 7th year and case was approved recently.

You must consult with an attorney but from what you mentioned in your note, you should have no problem to stay legally till June as once your status changes to H-1 you are no longer dependent on your husband\'s status.

About the medical leave part I am not so sure how that will figure out and you should definitely consult with your attorney. To best of my knowledge any time spent outside US can be extended.

I was told by my attorney, that for the time spent outside US during H-1 6 years you can certainly extend your H-1 status provided you are able to present documented proof for the stay abroad like tickets, credit card receipts etc.
