Initiating my N-400 process. Have a question.

Hi all,
It's finally time for me to become an American. I've been a LPR since 4/2002 and since my GC is expiring next year I'm going to go ahead and just apply for Citizenship.
I've started gathering all the documents I need, and so far I have almost everything to send out with my application but one thing. They ask for a certification letter from my son's mother that I've been paying timely child support, since is not court-ordered, I can't show any official order, and going back to print 7 years worth of check records is not my first choice.
So I wonder if any of you guys can share a template or form on how to write this letter.? I have been searching online but I haven't found a good example. Any help/pointers will be much appreciated.
This is basically the only thing I have left in order to send the app. out.

You don't need anything fancy. A simple statement by her saying that you provide support as agreed by the two of you is sufficient.

The affidavit must be sworn to or affirmed by the affiant (her) and include the name and address of the affiant, and the relationship of the affiant to the N-400 applicant (you). Each page of the sworn statement should contain the applicant’s A-number and for you the N-400 receipt number would help keep it with the application. You probably only need a single page.

It should be signed under penalty of perjury and end with something like the language used on USCIS standard sworn statements:


I certify, under penalty of perjury under the laws of the United States of America, that the foregoing is true and correct. I understand that information contained in or pertaining to any application shall not be disclosed without my written consent, except as permitted by law or at the discretion of the Secretary of Homeland Security.


Printed/typed name of affiant: __________________________________________ Signature of Affiant: __________________________


Sworn/Affirmed and signed under penalty of perjury on this __________day of ______________________, _______________.

Question about Part 10, Section D. Good Moral Character

Hi again,
On question #16, (Have you ever been arrested, cited, etc,) I have answered YES, due to three traffic violations, do I answer YES as well on question #17 (Have you ever been charged of committing any crime or offense?) I've never been charged of committing a crime, but traffic violations are considered offenses right?
