question about authorized stayed date on I-130

I have a couple of questions about the date entry on the I-130 form. My wife came to US with a student visa which expired June 2011. She graduated from the school in May 2011. We got married in Aug. 2011. Her OPT is extended to June of 2012.

On the I-130 form, number 14 question, it asks for arrived/departure record(i-94) and the date of arrived. She took a trip with school to Canada for one day. The date on the one day visa is April 8, 2011. Should I use that date or the date from the white card(i-94) that is stapled to the passport? Also it asks for the authorized stayed date on I-94 or I-95. What should I put in as the date? On the I-94, the officer wrote F1 D/S. Do I put June 2012 or May 2011 as the date?

They want the latest entry date, so that would be April 8, 2011. Write D/S for the authorized stay date, since that is what is on the I-94.

If she didn't get a new I-94 when entering in April, attach a cover letter to explain that she took a trip to Canada on that date, but didn't receive a new I-94 because it was such a short trip, and that her last entry before that was on x/x/xxxx at the port of entry xxxxxxxxx. Make sure to include a copy of the passport page with the Canadian visa and arrival stamp.

D/S means duration of status. It is the old annotation but is still generally used BUT the actual status is really now controlled by the I-120 as produced by and as reflected in SEVIS. If USCIS wants to know, they can look it up.
