What is a green medical exam slip?

Hi guys, I am about for file an I-485 and I feel a little confused about the I-693 form. Do I really need to fill it out? I have a green medical exam slip that was given to me by the doctor a couple of years ago. Will that suffice? What is that slip? Thank you!

Thanks for your prompt response! But are you sure that the yellow slip is not an I-693? Maybe they are the same thing?

When I did mine in 2008, I never got any slips. I got the results sealed in an envelope. However, I did request a copy of the results and they gave me one with the word "copy" stamped.

The color of the slip is actually green. Sorry!

Okay, I've looked up that slip and it turns out it's green, not yellow. I am attaching a scanned copy of it. Please take a look and tell me you opinion as to whether it can substitute that I-693 form. Thank you!
