Can Someone Explain, please?

I am USC,concurrently filed I-130, I-485 and I-131 for my mother. I accompanied her to interview on August 16, 2010, officer approved adjustment of status application on the spot.

Since then we have received approval for I-130 and I-131, but not no welcome letter on I-485. Most of the post I have seen I-130 and I-485 are approved at the same time usually approval letter for both I-130 and I-485 arrive at the same time. For my mother it is been about two weeks since we received approval for I-130 but nothing for I-485.I do not see any online updates for all application, they all are stuck at initial review stage.

Is it normal that I-130 and I-485 may be approved at different times?
Is it possible that officer forget to order card or approve I-485 on system?
Any experiences or suggestions?
 
Nothing is normal about USCIS, that's an anathema. You have nothing to worry about, you have 30 days after the interview for USCIS to mail you the card and update the status online. If the IO said she's approved, take his word for it. IS your mother traveling anywhere soon and this impedes her travel plans? If the answer is no, then just relax....all will be well...

Was that a purely verbal approval, or did he give her anything on paper showing the approval or stamp anything in her passport?

My mother was given a generic approval letter with congrats your AOS application is approved checked. officer said she is going to order ARC same day.

from all other post it seems we should have got welcome letter with I-130 approval?

I wouldn't sweat the lack of welcome letter. Expect the card within a few weeks.
