If you have proof of financial cohabitation, forget the affidavits.

we just have the joint bank accounts so thats y i wanna send some affidavits with that

In that case, there is no standard template. Just to let you know, if the only financial cohabitation proof is a single bank account, affidavits might not be enough. Don't you have common auto, medical insurance, joint credit cards, etc???

we have joint bank account,health insurance and we dont have joint auto insurance cause i dont have the drivers license
