can an i 485 approved i 131 denied? URGENT

my i 485 was approved i 131 denied om the same date, can this happen? what does it mean, i get my green card and cant travel, or is it that since the green card was approved the travel document is not relevant?
 
I believe, if your 485 is approved then you no longer need 131. Probably, this is the reason they denied your 131. Someone else confirm this please.
