Stolen Green Card while not in US

Dear experts,

I apologize for creating this new thread although there is similar one - but not the same situation.

Do you think you could help me:
My wife and I received (adjustment of status) employment based green card (08/28/08). After that we decided to travel to Europe - where my wife's wallet has been stolen - with the green card in it. I know we will have to apply for the green card replacement (form I-90) after we will return to US.

My problem is - I am not exactly sure how to answer some particular questions/fields of the form I-90 correctly:

* City of Residence where you applied for an Immigrant Visa or Adjustment of Status
Is it the city where we have been living at the time of filing Adjustment of status(from I-485 form)?

* Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted
We have filed to Nebraska office - should I answer "Nebraska service center"?

* Date of Admission as an immigrant or Adjustment of Status
Is this the same date as "Resident since: ..." mentioned directly at the green card?

* If you entered the U.S. with an Immigrant Visa, also complete the following:
Does this even apply to us?

A. Destination in U.S. at time of Admission
B. Port of Entry where Admitted to U.S.



I know - these questions may seem trivial to you - but I really want to make sure I make no mistakes.

Not if you came to the US as a non-immigrant and then did AOS

A. Destination in U.S. at time of Admission
B. Port of Entry where Admitted to U.S.



I know - these questions may seem trivial to you - but I really want to make sure I make no mistakes.

By the way, are you back in the US? Your wife was able to get back in, I assume.
 
Well I was confused by the exact same questions you should look in my thread just a couple below this one. But this is what I was told:

* Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted

Yes that looks correct

* Date of Admission as an immigrant or Adjustment of Status
Is this the same date as "Resident since: ..." mentioned directly at the green card?
YES

* If you entered the U.S. with an Immigrant Visa, also complete the following:
Does this even apply to us?
No this does not apply to you if you entered the country under different terms and became a permanent resident while living here.

Well, we're still in Europe - but getting transportation document which will enable my wife to arrive to US shouldn't be any problem.

No this does not apply to you if you entered the country under different terms and became a permanent resident while living here.

With this being said - I do not understand question "Destination in U.S. at time of Admission" which belongs under "If you entered the U.S. with an Immigrant Visa, also complete the following:" What could have possibly be "destination in US" for somebody who had not been in US at the time of Admission? Or - what is it "time of admission"? Am I missing something?
 
Hm - actually - more I think about my previous post - that question isn't really complicated. And moreover - it doesn't apply to my case anyway. So - thank you again for your help.

Yeah you can just ignore that question as well. Everything under the immigration visa section.

Are you going to e-file? I would be curious to keep track of your status since I just efiled for my wife yesterday. Might be helpful to know each others timelines on how fast things get done if we both keep updating our status here.
