Lost oath ceremony letter, what to do?

This is my first post, I'm not sure where to post it,but here goes. My husband just recently had his interview at garden city for citizenship 11/13/08 and got approved, he received his oath ceremony date 2 weeks later. his date is next tuesday, he took the letter to his job for the day off and lost it, his job made a copy but there is still the questions in the back about travel etc. what should we do? use the copy to attend or is there a way to get a new letter before the ceremony date or should we get an infopass?

Thank you so much, you are a life saver, I was starting to freak out. so can we staple the back portion to the copy
