I just have my EAD as a backup, meaning - I am still on valid H1. In this case, do I really need to file for EAD renewal strictly 90 days before its expiration date?

No, in fact you don't need to renew it at all if you don't want to... You can do it any time...

EAD and AP are optional, not a must. Apply if you want them and have an extra dollars to spend without reason.

is there a policy to renew 90 days before expiration? My wife is using EAD, and I did not apply 90 days prior to expiration.. will there be any issues?

The only role is not to apply more than 120 days before expiration. It is up to you, the point is to take the maximum time allowed to secure its issuance before expiration of the current one especially if you are using it.
