If the birth is going to be registered after marriage, should the birth certificate be under maiden name or the name after marriage?
------------ it should have name that was your name before marriage
Can somebody please answer?
FYI If your birth is late registration then you will need two affidavits from your parents and the affidavits should have the info of your mother name, father name, date of birth, place of birth, your name at the time of birth, it should also state that your name was XYZ before your marriage to ABCD and your name was changed later after marriage to HHHHHH.

*DOB, Place of birth should match with the info on your passport

if you are from India then Affidavits should be on stamp papers signed by your parents before Notary or executive magistrate.
If your birth certificate is not in English then you will also need it translated by Notary .

**** Why don’t you just get non availability of birth certificate and two affidavits from Parents??????

thanks for the reply

Can you point me to sample affidavit for the parents?

Yes from India. What kind of stamp paper should be used for this affidavit?

thanks again.

Yes from India.

------------------Lawyer in India or notary knows. i think affidavit should be on 20 rupees or 10 rupees stamp paper.
thanks again.
FYI Affidavit should cover. The name of your father or mother or who is giving the affidavit and relationship with you, relation ship with you, your name at birth where you were born to your mother… wife of… date of birth place of birth, you name before marriage was… you got married to XXX on date … and later your name was changed to BBBB

**** If the non-availability of birth certificate from Municipality or registrar of birth or village Panchayat is not in English then also get it translated in English by notary
