Query reg. H4 visa

Hi,
I have come to US for 1st time a few months back on a H1B visa. My wife also joined me here on H4 visa.

I want advice on the following:

My wife's maiden name is "SHIKHA" with no SURNAME.
In her passport her name is entered as:
Given name: SHIKHA
Surname: blank

So, in her H4 visa, her name is mentioned as
Given Name:"FNU"
FamilyName:"SHIKHA"

We had applied for a Driving license for her, but the DMV refused to give license as they need both names (first name & surname) for issuing license, & they are not considering FNU

So, we want to get rid of this "FNU" from her passport & visa. We are thinking of adding my surname (SINGH) to her name ie her name will be SHIKHA SINGH

1) Whats the process of changing name in passport after coming to US?

2) Whats the process of transfering visa from old passport to a new passport?

3) Will there be any issue if she gets a new passport with a new name, & the visa on her old passport remains with her old name?

4) Will her immigration status in US will remain valid?

------------------------FYI SINGH is also not surname in North India many are SINGH but SINGH is not Surname in Punjab all have SINGH now they are supposed to write SURNAME on Passports. RAJINDER SINGH will write RAJINDER SINGH GILL, BALJINDER SINGH will write BALJINDER SINGH SANDHU

------------------------Contact the Indian consulate or check nearest Indian Consulate and read the info, she need to fill form and may also have to Publish the advertisement. has she got your name added on passport as Husband

2) Whats the process of transfering visa from old passport to a new passport?
-----------------only US consulate can transfer
3) Will there be any issue if she gets a new passport with a new name, & the visa on her old passport remains with her old name?
-----------------I think she will face the same problem as DMV will need same info on Passport, Visa stamp and her I-94. Talk to DMV Supervisor and get the more info if she change her name on passport will they give her DL
4) Will her immigration status in US will remain valid?
