Unfortunately I don't think my case will be of much help to you. I sent in my N-400 nearly 5 years AFTER I became eligible. In fact my GC was set to expire in the middle of this year.

Yes (Baltimore DO) Yes. I had my picture, and signature, taken - and those were the ones used on the Naturalization certificate (not the ones I sent with the N-400 form).

I went to the local IRS office and got transcripts for the last 3 years. The IO never asked for them though.

Waited 5 years after being eligible... And only because it was a pain to get visas to travel.

Just answer NO... You'll be fine.

And take copies... I was asked to submit a copy of one of my originals and when I said I did not have it the IO wasn't too happy. After some grumbling she made the copies herself.
