Hello. Question 29 on DS230 II asks for the name and address of a petitioner. In the case of diversity application, should we put N/A here or should we type the name of the applicant. Thank you

Is the rank order number (mentioned in form DSP-122) the DV case number. i.e 2008AF00002xxx? Also, when we talk about the upper right hand corner of each form is this the right hand corner as we look at it? i.e. to close a webpage you click the "X" in the upper right hand corner? Thanks a lot.
