Wife birth certificate: Name \'Nil\' question

Hi,
My wife\'s birth certificate says her name as \'Nil\'. which means she was not named at the time of her birth. What kindof affidavit do we need to get
for her from her parents in India. Can someone please share any info on this. I heard that it is an affidavit done at a notary public on a Rs. 10
stamp paper. Can someone please also tell us who this notary can be.
Other than her name as \'Nil\', everything else is fine on her birth certificate. Please help us.

Two affidavits from one from your wife\'s mother and one from her father would be good enough, where ever you buy that Rs. 10 stamp paper you would find notary who would type it and give it to you. The Rs. 10 stamppaper should be obtained in your wife\'s mother and father\'s name separately and with the affidavits content typed on it.

Does it need to be signed by any lawyer or megistrate, I mean the affidavit. Can someone explain more on this please.

Can somebody please post the what the content of the affidavit should say.

Is notarty public a lawyer or someone like that. Can somebody please
explain this for us.
