Which form to fill out to update social security records?

This is same form as first time applicant. It says they will need original Naturalization certificate. Do they just need to see that or will they actually keep it while they process my application?

Once they make a copy - you will get back your original certificate. You can go with your US passport instead of NC - and they will accept it.
