where to send vaccination proof for second td vaccine

My father submitted his application for permanent residency a couple of months ago. Since then he has received Advance Parole and a work permit. No Green Card yet. In the meantime, he got his second TD vaccination (the doctor said that he needs three of them - one before filing, one - 2 months later, and another - 6 months later). Where do I send the paperwork from the doctor about this second vaccination?
