Where to go to get iEAD in San Jose area?

Can someone confirm the address where to go to get the iEAD? I found this on the internet:

Also, what is the best time to go there in order to get the iEAD without having to wait in the line for too long? Basically the best algorithm to get the work done in minimum time.

Someone posted on this site that we should also carry the state Id.

All I have is my wife\'s passport. Do I have to first get the state Id for her before we go for iEAD?
