where do i find formats for affidavits in lieu of Birth certificates??

Satmat here is the format I used for affidavite

AFFIDAVIT BY name of parents

Re: name of child

We, (name of parents), hereby declare that :
1. Our full and complete address is
2. That I, (name of father) was born on (date-of-birth) in the (place of birth), India.
3. That I, (name of mother) was born on (date-of-birth) in the (place of birth), India.
4. We married on (date and place of mariage), India.
5. We (name of parents) had the following children:
a. (Child\'s Maiden Name): born on 9date of birth) and
b. (names of other child/ren)
6. (name of child) was born in (place of birth), India.
7. This affidavit is being submitted because there is no official record of birth for (name of child) in (name and place of birth record office).

We declare under penalty of perjury, that the foregoing is a true and correct statement.

Before whom should it be sworn. Like Magisterate or Notarary or Lawyer etc.

What about non-availability

I heard that we should submit a non-availability of birth records from the local governing office (like muncipality etc.). Do you have a format for that?
