when to apply for H1 Extention for the same employer

I am an Indian and holding valid H1B visa. Now I am in USA and working here. My employer petition got approved in 2006 which is valid from 10/02/2006 to 10/01/2009. I can see this in the Form I797. I attended an interview in US ambessey, Chennai, India in 2006. They wanted to go for the further administration processing and they given 221g card. After that they did not give me the visa untill July,2008 (After two years). So I got the visa stamped on my passport in July, 2008. In the passport they mentioned Issue Date as 07 JUL 2008 and Expiration Date as 01 OCT 2010. My question is : When I need to apply for the H1B extention ??? petition expiry date on Form I797B is 01 OCT 2009 and visa expiry date on passport for the same petitioner is 01 OCT 2010. I will be thankful if some body let me know whether I need to apply for the H1B extention before OCT 2009 or before OCT 2010 for the same petitioner.
