When is the due date to mail back the form?

Sorry to write another topic, but i just want to make sure because i found there's another form which i don't know if i should mail back to my country's US Embassy, it's a checklist form, but i'd appreciated if someone tells me if i should mail back to my country's US Embassy. Besides can i know when is the due date to mail back the form to my country's US Embassy? Thanks

Checklist form

I have read it carefully and it says(and probably not)to mail one checklist form back to my country's US Embassy, the checklist form contains a list of documents that they require, have you receive this kind of checklist form? and the form has my country's US Embassy address on the top right hand corner. If you have similar form please do let me know. Thanks. Besides how to know if my information filled up correctly? how to know if i have all the forms(DS230 Part1,2 and DSP-122, bar code form, photo) they need? would they let me know or would i need to call them and check if they have all my forms(all they need) to pass me to the interview?

I'm worried, don't know if i should mail that form to KCC because i was told by my country's US Embassy via email that i need to keep it for interview but after i read it over and over i found suspicious i have no idea if i should mail it to KCC. Please advise thanks
