When fill out I-765, should I apply "Renewal of my permission to accept employment", since I have applied to EAC (employment authorization card) before. That\'s when I did my practical training.

Or should I apply "Permission to accept employment".

No.. Not renewal..

You have to mention "RENEWAL" only if you are renewing an existing EAD. Since pratical training was valid for an year and has expired long time ago, you have to select "Application to accept employment". In the I-765 application there is a section where they ask you whether you had an EAD before, that is where you have to mention about the Practical Training. Include a copy of the EAD (both front and back) of the Practical Training EAD card.
