What to take to Doctor's office

Do i need to take letter from INS about RFE to doctor's office and what about asylume approval letter.
Also i don't have list of vaccinations taken back home. I see there is a long list of vaccinations requirements. Seems like he would run out of room to inject so many syringes into my body?
Please advice?

You no need to take letter from INS or Asylum approval letter, but make sure you take valid picture ID (Driving Licence, State ID). My doctor asked me for passport but I said I don't have it.

For Vaccine, there are generally 3 , First is TD which they say you have to take every 10 years. for ther 2 vaccines (MMR & ?) doctor will do the blood test unless you have the proof of taken. In my case he gave me TD and MMR.

Also doctor will give you Tuberculosis skin test on first visit, which you have go back in 2 days (they will tell you when you need to come back). If this test come positive then you have to go for X-ray (even doctor has little doubt he or she will send you for x-ray. it happend to me but in x-ray everything was normal).

Once x-ray report comes normal your papare will be ready to picked up, but don't forget the fee ;)
