What should I Do

I did not receive my Green Card after the change of address. The GC stamp expires in a month. Should I file I-90 form at Local INS..?

Please help...!!!!
 
Some More info

I mailed the change of address and saved the copy of the mail receipt.
I was never able to reach an IIO. I am in East Cost and my app. service center is CA
