what should do that my J1 status form missed?

Hi,

My certification form J1 status is missed by mistake, but all other forms are okey. What should I do? Is it ok to return without this form.

what do you mean - missed? Where are you returning to? Details, please.

You can request a copy of DS-2019 from your sponsor.
 
I mean lost the J1 status form. I have gotten the J1 visa, and already into US form China. and now I wanna return to China, fortunately, I do not lost I-94 card. So i'm not sure whether I need J1 status form when come back. Thank you!
 
no, you do not need DS-2019 when you are leaving the US. BUt for the future (you never know when you are going to need it -- I needed my DS-2019 10 years after my exchange program), I would advise you to get a copy of your DS-2019 from your sponsor anyway. Especially, if you are just briefly travelling to China and are coming back to the US for the same program. You will need your DS-2019 if you plan to reenter US for the same program.
 
Thank you much for your clarification, I have got a copy of DS2019 form. I just concern that I must return back befor the date in DS2019 form, so if I lost the form, it may have some issues.

Hi LucyMo,

I am now in exactly this situation that I need my former IAP 66 but my former sposnsor (aupaor agency0 does not have ita ny more (nor do I).

They will sned me letter that they only have to keep files for 3 years, since it is 10 years ago, so I hope it will eb okay but I was another time again an auapir and this organisation does not wrote me back at all.
ANy advice, still a chance for waiving my J1 or is the Dos that strict??
