You need to send I-765
2 ADIT pictures
Copy of I485 receipt notice
Copy of old EAD
Counter Foil from Previous EAD Jacket
Renewal Fees of $120.00

The mailing Address is

INS,VSC
75 lower welden st
Saint Albans, VT 05479-0001.
