what is the address for application of AP in VSC

could you guys pls give me info about all the documents needed for the application of AP. if it is extension...then should i send the photocopy of previous AP??? Is there any letter required stating the reason of leaving the country??...pls help me ..

75 Lower Welden Street

fill the form: you may get a fillable form on the INS website
prepare a check for fees
no need to send earlier AP copy
take 4 ADIT style photos (please check exact number)

also, attach letter that you need multiple entry due to travel related to job(if applicable).
