what day do I become eligible to apply?

My GC says "Permanent resident since 11/3/04"
My approval stamp in my passport says "DOE 11/12/04"

So obviously the 2 dates don't match!!

What should I do?

Also can someone tell the evaluation date they consider is the date you enter in your form OR the day they RECEIVE.

Use green card's date. that's is your date that you became a US permanent resident. Make sure to make copies of your green card and attach it to your n400 when you send it to the immigration office.

If you were not in the US on 11/3/04, but you got that approval stamp on 11/12/04 at the port of entry, use the 11/12/04 date to calculate when you can apply, because that is the date you were first admitted as a permanent resident.

And since it's only 9 days difference, just use the later date anyway. Even if the earlier date is technically correct, you don't want USCIS erroneously rejecting your case before the interview because some clerk is looking at the 11/12 date.
