What are the documents required for applying helth Card in Ontario?

I am planking to come to USA to complete my Canada Landing formalities in 15 days (Including apply for SIN Card, Health Card in ON, Open Bank Account etc)

Please advice me what are the documents required for applying for health card in ON.

My friend telling that only one Bank statement will be sufficient to apply for health card.

Please advice me.
