want to request for i 485 receipt notice copy

I have applied for I 140, I 1485 mine and I 485 (spouse) on December, 2003 in first week.
I have received only two receipts of I 140 and I 485 of (spouse). Mine yet not come .My company attorney said he applied for duplicate 05 months back.
He says no response. He is very difficult attorney to deal with.

Now I want to take charge request for duplicate copy of my I 485. Pl advise me what is the procedure.
Q1) Can I directly apply to USCIS for duplicate copy?
Q2) Any form ,Letter,Content,Fees ,Procedure?
Q3) USCIS will send that copy to me or to my company attorney again?

Call the 1-800 number and Request for a duplicate copy. They will check your address and name. They will send you a copy of it. I think they willl send a copy to your attorney.
